After getting the names of your group to be prayed for, they can be scanned in (scan as document, save as OCR editable text, can open or copy to Word) or typed onto prayer cards.
Type up prayer suggestions that can be inserted in the back of the prayer cards. You can tell your people who are praying that they can use the prayer suggestions or their own prayers.
Put the sheets in a 4x6 vinyl passport envelope which can be obtained through office supply stores or online. (wilson jones makes one).

You can start by collecting emails of potential people through existing email lists, facebook, or text. An introductory / invitation email can be sent out telling about Project Prayer and asking them to reply back with their name and email, (address, cell number optional) if they would like to participate. Email us at ProjectPrayer@yahoo.com if you would like us to email you an example introductory email.
You can also get people and form groups through facebook or other social media. You just need a way to communicate to them on how to get their prayer card and when there is a prayer card exchange.
You can send out an email or Facebook post or other form of communication and let the people know how they can get their prayer card. You can set a time and place such as a coffee shop or group meeting or other activity for them to get them. You can also mail the prayer cards out to those who can't pick theirs up.
You can ask the members to pray for their list of people as their schedule allows. Even if the person can only pray once a month for each one on their list, the impact can be huge. Also, many times the prayer cards have to be duplicated because of the number of people who want to participate, so more than one person is praying for the same list of names.
You can have Prayer Card Exchanges where you let your praying members know of a certain time and place for them to meet and exchange their prayer card with another person. They will then get a new list of names. Before they exchange, they can take out the front sheet of their prayer card which has the list of names on it. On the back, they can sign their name and put the dates for which they prayed for their people (ex Sep 2012 - Apr 2013). You can have exchanges as often as you would like or not have them at all. A common thing for schools is twice a year, once in the fall and once in the spring. If you do, it is special to see the list of signatures and dates of all who have prayed for a certain list when the students graduate or the people are no longer in the group.
Because of the quick communication through email or facebook, you can have people let the organizer of the group know of any urgent prayer requests. The requests can be sent out to all the praying members who can pray for the person or situation immediately.
For large school districts, we have found that starting up Project Prayer with the 8th grade class is a good time. This gets all prayer cards out before the students enter high school and definitely need a lot of prayer.
As new students move into the school, each summer the yearbook can be used to get the new students' names and to print out a few more new prayer cards. Those prayer cards can be passed out at the fall prayer card exchange to get them into circulation.
Don't worry if you are praying for students who have left. They still need our prayers! An option is to "redo" all the prayer cards the summer before their Junior year, dropping off all the students who have left and getting a more accurate list. At the fall swap you can have everyone turn in their old prayer cards and pick up their new more accurate one. You can reuse the old prayer card holders.
At Graduation:
An idea at high school graduation is to give all the students a Bible Promise Book with a Letter of Encouragement. The letter can say: congratulations, we are proud of you, you have wonderful futures ahead, we have been praying for you since the 8th grade. On the back can be a copy of all the signatures from the people who have been praying for them over the years. You can start collecting signatures on a sheet of paper their Senior year. Also, a great Bible scripture to put on the letter is:
"For I know the plans I have for you," declares the Lord, "plans to prosper you and not to harm you, plans to give you hope and a future." Jeremiah 29:11
